Update Annual Student Verification Forms

Verification of legal settlement/ability to enroll must be provided by a parent/legal guardian of a student seeking enrollment through the following documentation:
Online applications WILL NOT BE APPROVED until all required documentation has been provided.

Legal Guardian Identification

Driver’s License
Passport/Visa
Military ID
State ID

Proof of Residency #1

ONE OF THE FOLLOWING:

Deed
Building Permit
Dated Rental Agreement
Mortgage Statement
Current Lease in its entirety (must be a full copy that includes all pages, lease term, address of the leased property, name and phone number of the lessor, name of the lessee, and a listing of tenants)
Property Tax Statement

*Required from Parent submitting a VOR:

Auto Insurance Bill
Bank Statement
Government Agency Letter
Pay Stub
Medical Bill (hospital, clinic, insurance)

**All documents must be current, include
parent/legal guardian’s name, and list an address
within FTCSC borders.

*If, and only if, the parent/legal guardian and student are living with another family member or individual residing in the Corporation’s geographic boundaries, and the parent/legal guardian cannot verify residency through #1, the parent/legal guardian can provide a sworn statement, via a verification of residency form, from the legal resident of the home in the Corporation’s boundary attesting to the Parent/legal guardian and Student’s residency at the address in the Corporation boundaries. The parent/legal guardian is still required to verify residency through #2 above.
